Impact on In-home Monitoring for CHF on Re-hospitalization Rates
Congestive heart failure (CHF) is one of the major cardiovascular diseases with high global incidence and prevalence in the United States. While numerous advances in evidence-based medical therapy continues to occur, congestive heart failure remains a major problem as it results in significant burden of mortality, morbidity, and costs. The United States has a prevalence of 5.8 million individuals with the condition and more than 960,000 cases annually. It is estimated that approximately one million hospitalizations linked to congestive heart failure occur annually. Most of these hospitalizations arise from worsened congestion among already diagnosed patients. The United States spends approximately $32.7 billion on congestive heart failure annually. Introduction
The functions of the historical state correctional system have changed since the founding of the nation more than 200 years ago. The Jacksonian Era, the Era of Reconstruction and the Progressive Era on up to the reform of the 1970s all effected different changes to the function—i.e., the goals and activities—of the correctional system. Pennsylvania’s state correctional system was the first to introduce solitary confinement as a way of removing the deviant element from society. This was part of William Penn’s attempt at social reform, a fundamental characteristic of his Quaker ideals (Fantel, 1974). The goal of Penn’s plan was to promote moral rehabilitation and it was believed that through the inmate’s lengthy time alone to reflect on his misdeeds he would begin to develop the moral resolve to reform himself and become a better contributing member of society. Although the precise causes differ from organization to organization, the high costs that are associated with unplanned turnover are well documented. In fact, in some professions, replacement costs for employees can easily equal their annual salary or even more, so identifying strategies to reduce turnover rates represents a valuable and timely enterprise. To this end, the purpose of this proposal is to provide a description of the problems of interest and what can be done to mitigate these issues. A description of the implications of implementing the career management program is followed by the key recommendation that emerged from the research. These recommendations include providing employees with a career management plan that motivates them to acquire additional education and training as well as instilling an enhanced sense of organizational loyalty to reduce turnover levels. In 1981, the U.S. Congress declared October as Domestic Violence Awareness Month, marking a celebratory hallmark for advocates and survivors nationwide (National Resource Center on Domestic Violence, 2012). Since this time, similar social and legislative initiatives have increased overall awareness of gender inequality, thus influencing a decline in women\\\\\\'s risk for intimate partner violence (IPV; Powers & Kaukinen, 2012). Recent initiatives, such as a national briefing focused on brain injury and domestic violence hosted by the Congressional Brain Injury Task Force, continue to call increased attention to the various intersections and implications of this national public health epidemic (Brain Injury Association of America, 2017). Unfortunately, despite various social advocacy movements, IPV remains an underrepresented problem in the United States (Chapman & Monk, 2015). As a result, IPV and related mental and physical health consequences continue to exist at alarmingly high rates (Chapman & Monk, 2015).
