Advocacy in health care refers to the action of promoting and fostering social, economical, political and educational changes that reduce the risk of suffering of individuals and communities by implementing preventive strategies, increasing health literacy, and boosting access to care and health equity (Earnest, Wong & Federico, 2010). Health care advocacy agents can thus address health outcomes by advocating for changes to the current practices, environments, awareness, and access to care that populations face. Ageism, isolation, and misconceptions about gerontology are all issues that the vulnerable elderly population faces (Brojeni, Ilali, Taraghi & Mousavinasab, 2019). As Yamada et al. (2015) point out, however, the issue of healthcare disparity and healthcare inequality of the elderly is one of the biggest issues because it underlines all the others: if the elderly population does not even have sufficient access to care, it is unlikely to be able to battle the effects of ageism, isolation or ignorance about gerontology. Healthcare costs have doubled since the 1990s (Yamada et al., 2015), and this puts an added burden on the elderly population, whose resources are finite. Yamada et al. (2015) put it this way: “as healthcare costs rises, more of the increasing costs are transferred to certain disadvantaged populations, and patients would have to spend a considerable share of their healthcare costs out-of-pocket” (p. 1745). Introduction
This is part 3 of the quality and sustainability paper. Part 1 outlined the descriptive method of assessing quality and safety measures in nursing science. It was concluded that administrative data could be used to determine the effectiveness of quality or safety program since safety is the most fundamental aspect of nursing science (Twigg, Pugh, Gelder, & Myers, 2016). In part 2, analysis and application of safety programs were explored and Kenneth E. Waldenberg Health Care facility was used as a model in the analysis. The findings of the paper was that leadership plays a key role in sustaining quality improvement programs in any health care organization (Stetler, Ritchie, Rycroft-Malone, & Charns, 2014). In this third part of the quality and sustainability paper, a change theory that will support the implementation of a quality and safety program will be discussed.

Two provisions in the PPACA (Patient Protection and Affordable Care Act) that impact my current nursing practice are 1) the call for increased access to care and 2) the call for more preventive care. The Institute of Medicine (IOM) in its Future of Nursing report stated that nurse practitioners should be allowed to practice to the full scope of their education and training (IOM, 2010), which is something they are not permitted to do in every state. Many states require nurses to practice under the supervision of a physician, but as O’Brien (2003) pointed out, Advanced Practice Nurse Practitioners (APRNs) were trained to take the place of the physicians who were leaving the primary care field for specialized medicine. The Institute of Medicine (IOM) in its Future of Nursing report stated that there is a need for nurses to be able to practice to the full extent and scope of their education and training (IOM, 2010). In doing so, the belief was that this would increase access to care and provide nurses with the opportunity to practice as they were trained. O’Brien (2003) notes, after all, that the whole reason APRNs began being trained in the latter half of the 20th century was to fill the gap being left behind by primary care physicians as they left their primary care practice to go work for specialized medicine. Today, however, nurses are still bound by regulations that require them in many states to work under the supervision of physicians even though the nurses have the training to treat patients independently of doctors.

...Caring My specialty area of NP practice is family nurse practitioner. My choice of nursing theory on this front is the Dorothea Orem’s self-care deficit theory. To a large extent, this happens to be one of the most popular nursing theories owing to its succinct description of some concepts of great relevance to nursing practice. In essence, the theory advances the need for individuals as well as families to further their health and wellbeing by embracing the various practices associated with not only health maintenance, but also health promotion.
