Prevention: Primary, Secondary, Tertiary (diet, exercise, immunizations, screenings)

The Health Belief Model would contribute to a lesson in prevention of the condition's worsening severity. Here, the primary level of prevention is seen as dietary. The secondary level is seen as exercise and the tertiary level is viewed as immunizations and screenings.

Teaching Strategies

Teaching strategies will included the use of visual aids and literature materials as well as the applied discussion of realistic dietary and exercise goals.

Resources

Resources to be used in supplement to the education of the patient will include pamphlets regarding the implications of diabetes, the distribution of kits with required self-treatment materials, the compilation of useful web resources, nutritional dietary recipe kits and preliminary exercise regimen diagrams.

Methods of Evaluation

The effectiveness of the proposed teaching learning plan would be evaluated according to key health indicators identified in the subject. In the case of diabetes, these would be such features as body weight, body mass index, blood serum levels, blood pressure and cholesterol.

Implementation:

This lesson plan would be carried out upon scheduled visit to a private physician's office.

Date, site and time:

Over the course of a one hour appointment scheduled for noon on April 15th, the subject would be provided with detailed instruction and the aforementioned materials relating to her condition.

Participation Data:

The patient would be alone with a small core of physicians during the consultation.

Evaluation:

Pt response and level of understanding

The patient demonstrates distress over the changes in diet and exercise, indicating this to be a significant deviation from existing lifestyle patterns.

Pt knowledge, behavior, attitude, skills

The patient shows a limited understanding of nutritional dietary requirements and therefore must adopt the skills to recognize and pursue healthier eating habits.

Teaching/Learning Outcomes

It is expected that the consultation described by this lesson plan would induce a belief that a greater control over diet and exercise will produce a greater capacity to combat the condition of diabetes.

Recommendations for Future Education/Reinforcement:

With respect to future monitoring, the patient will be instructed to follow up with the doctor every 3 months in order to have her vital signs and lifestyle habits assessed.
