Medicare Access And CHIP Reauthorization Act
...Obamacare The Medicare Access and CHIP Reauthorization Act (MACRA) changed the way health care providers are reimbursed through Medicare, provided an increase in funding, and extended the Children’s Health Insurance Program (CHIP). MACRA placed conditions upon care providers in order for them to receive reimbursement: care must be quality care, i.e., care that helps them to keep from having to come back for more treatment—rather than just treatment after treatment, keeping patients coming back through the revolving door of medicine without every actually helping them. The type of quality care required by MACRA is preventive medicine and health literacy promotion. As Licthenfeld (2011) pointed out, too many doctors are diagnosing patients with health problems that are insignificant and do not need treatment—but because Medicare was always willing to bankroll treatment it made good business sense to treat every patient for every symptom. The healthcare sector in the United States is facing shortage of healthcare providers, particularly in rural settings. One of the most affected areas in the country is rural Maryland, especially Allegany County, which has been selected as the location for this project. The proposed plan seeks to help lessen the shortage of healthcare providers in the county using MedChi, an organization that provides public health resources in Maryland. MedChi could help address the problem through promoting adoption of technologies like telemedicine and expansion of the scope of practice of non-physician providers. To realize this objective, the organization needs to establish partnerships with relevant stakeholders within and outside the county’s health sector including policymakers. These partnerships should focus on identifying necessary resources for implementation of the proposed solution.

Karen Ann Quinlan attracted national attention following her slipping into a comma in 1975. In essence, the Quinlan case remains a key reference point in discussions revolving around the right to die. According to Drane (1994) Ms. Quinlan fell into a comma on the night of April 15, 1975 following her an evening birthday party of one of her friends. It was reported that during the party, Quinlan took a cocktail of alcoholic beverages. In additional to alcohol, it was also reported that she took a tranquilizer (Valium to be specific) (Drane, 1994). She promptly slid into a comma. Soon, her friends realized she was not breathing and called an ambulance which rushed her to Newton Memorial Hospital. Introduction
This is part 3 of the quality and sustainability paper. Part 1 outlined the descriptive method of assessing quality and safety measures in nursing science. It was concluded that administrative data could be used to determine the effectiveness of quality or safety program since safety is the most fundamental aspect of nursing science (Twigg, Pugh, Gelder, & Myers, 2016). In part 2, analysis and application of safety programs were explored and Kenneth E. Waldenberg Health Care facility was used as a model in the analysis. The findings of the paper was that leadership plays a key role in sustaining quality improvement programs in any health care organization (Stetler, Ritchie, Rycroft-Malone, & Charns, 2014). Ageism, isolation, and misconceptions about gerontology are all issues that the vulnerable elderly population faces (Brojeni, Ilali, Taraghi & Mousavinasab, 2019). As Yamada et al. (2015) point out, however, the issue of healthcare disparity and healthcare inequality of the elderly is one of the biggest issues because it underlines all the others: if the elderly population does not even have sufficient access to care, it is unlikely to be able to battle the effects of ageism, isolation or ignorance about gerontology. Healthcare costs have doubled since the 1990s (Yamada et al., 2015), and this puts an added burden on the elderly population, whose resources are finite. Yamada et al.
