Who is involved in the goal, what is the goal, where will it take place?
Self, Colleagues, Supervisors and Subordinates. The development of leadership comes with not only "doing," but in learning to listen, to find opportunities to exert leadership in a number of ways, and to use leadership theory to buttress a tool box of information that will help in a variety of situations.

Goals will be measured and achieved based on three factors: personal inventory of success or failure; feedback from stakeholders; feedback (written and verbal) from supervisors.

What resources/expert available to assist you with attaining your goal?
Bibliographic sources on leadership theory, use of contemporary nursing theories (Watson, Lenninger, etc.) on specific aspects of leadership behavior, feedback from stakeholders.
R: realistic
Is this goal something that is realistically obtainable in a professional practice?
Yes, not only is the goal reasonable and realistic, it is necessary in order to actualize and become a better nurse leader.
T: time bound
What specific dates or weeks will you accomplish each task of your project goal?
Goals will be set by week, and broader, overall goals by month or quarter. Leadership issues are an ongoing and evolving process, and because of the nature of healthcare there are a number of variables that occur on a regular basis that may influence the teaching and learning moments.

Using a variety of resources, I will enhance my leadership skills and specifically work towards becoming a more effective servant leader.
Plan of Action
Attributes needed to achieve the goal; timeline of when you plan on doing what; explain how, when, and where you are going to meet or talk with your expert; and identify specific resources you plan on using to help you achieve your goals.
Leaders can ease the process of change through servant leadership theory -- communication is key, getting feedback, assuring staff that this is an exciting time for the company. This can be done through newsletters, emails, bullentin boards -- all offering comments that should be addressed at a company-wide meeting."The servant leader's motivation is directed more at the personal growth of the follower, thus the servant leader's success is determined by the extent to which the follower moves towards self-actualization" (Kuzmenko, Montagno, & Smith, 2004). Will meet with staff weekly, supervisors monthly, mentor as needed, and stakeholders at least monthly.
