Immediately I would question the patient regarding her medical and family history, including questions related to whether or not this was the first time she has experienced these symptoms. I would also inquire about the patient’s recent travels because it is possible the symptoms point to an infectious disease or a food-borne illness. Whatever the situation, the patient must first replenish the depleted fluids and electrolytes immediately during the administration of diagnostic procedures and patient monitoring. I have been attracted to Portugal for most of my life, because I have some Portuguese ancestry. I was curious, for example, about how much of the culture of family has been influenced by this background, and what elements. But as I got older I also started just to take an interest in the country in general. Because of her military service, she was trained and disciplined in a culture renowned for praising strength and abhorring weakness. In the military, the code also centers on unity and spirit of mission. Soldiers who “betray” their fellow soldiers are seen as untrustworthy and can be ostracized and marginalized. Because Suzy was sexually assaulted while serving in the military, she did not report the assault, fearing that it would lead to her being labeled a bad soldier by the others. Instead, she attempted to cope with the assault and the trauma it caused her. In attempting to cope on her own, however, Suzy encountered depression, a loss of self-esteem and self-confidence; she developed a problem with substance abuse as well as with anxiety. The blues is described as a uniquely African American musical tradition, combining folk music, traditional work songs once sun by slaves, jazz, and other musical traditions to describe both personal suffering and to create an oral history of all individuals who have sung it. In August Wilson’s 1986 play The Piano Lesson, an heirloom piano comes to embody the blues tradition for its central protagonists Boy Willie and his sister Berniece. Whether to transform the piano into money, as Boy Willie wishes to do, or to save it, becomes symbolic of the role of the blues in African American history and society. The blues, like the piano itself, is problematic because of its ties to the history of slavery, but it cannot be nor should not be eradicated, given the loss this will create for the community.
